Leveraging Technology and Analytics to Address the Social Determinants of Health

Description: As health care increasingly focuses on addressing the social determinants of health (SDOH), it is essential to use applied data analytics and predictive modeling to both identify patients, members and employees in greatest need and to determine efficient and effective interventions that improve their health outcomes and reduce the cost of their care. By integrating ProMedica’s clinical excellence and analytic capabilities, ProMedica has focused on the needs of specific patient cohorts, benefiting from standardized interventions designed to improve health care outcomes. By applying data-informed workflows into front-end clinical settings, traditional structures of population health management and have been disrupted and a new, innovative model of precision population health, leading to improved health and well-being for patients and a reduction in overall cost has emerged.

Speaker:  Kate Sommerfeld is the President of the Social Determinants of Health Institute for ProMedica, a mission-driven, not-for-profit, health and well-being organization. Kate is responsible for shaping ProMedica’s work in the social determinants of health, including clinical integration, medical education and training and strategic community initiatives. Kate has Bachelors of Arts degrees in political science and child psychology from Purdue University and a MPA from BGSU.


Speaker:  Dr. Brian Miller
is the Chief Medical Officer for ProMedica, a non-profit, mission-based health and well-being company. Dr. Miller leads the clinical informatics strategy and provides system-level leadership for data analytics and social determinants of health. He received his medical degree from The Ohio State University College of Medicine and is a board certified Family Care Physician and has been in practice for more than 30 years.
