HCC Education for Inpatient and Outpatient CDI

Description: In the evolution to value-based care, understanding HCC concepts is vital, particularly for those accustomed to acute care coding and clinical documentation improvement. The speaker will discuss how this coding system works by highlighting the interaction of weights and overriding categories. An emphasis on medical record documentation will include a review of the problem list, past medical history, and medication list as well as other appropriate clinical and administrative criteria that is often missed.

The speaker will define the different HCC models and discuss the utilization of both inpatient and outpatient records to reflect the patient’s health risk. Since determining a patient’s risk adjustment factor (RAF) is the intended outcome, the speaker will guide CDI specialists in identifying common missed query opportunities that reflect appropriate RAF scores for patients.
