Description: With the shift to population health and use in quality metrics, the Hierarchical Condition Categories (HCC) model has become very important. CDI’s focus has traditionally been inpatient and CC-centric. This session will explain the HCC risk-adjustment model, prepare attendees to recognize HCC conditions, and give them the tools to elicit clarification from providers in the inpatient and outpatient arena.
