Common Coding Errors

Description: With 25 different companies, hundreds of hospitals/medical groups, and thousands of auditors and coders using our coding audit platform, we are able to pull out the most common coding mistakes from a large population. Karen will present the most commonly revised, added and deleted diagnoses, PCS procedure codes, CPT/HCPCS and modifier codes as well as the most commonly revised MS-DRGs and APCs across the inpatient, outpatient and Profee environment and will discuss trends and root causes of these coding audit findings.

Speaker: Karen Pickering, RHIT, CCS, is a 30-year veteran and expert in coding, auditing, and mentoring to healthcare facilities and Health Information Management professionals. She received her RHIT (Registered Health Information Technician) certificate in 1990 and her CCS (Certified Coding Specialist) in 1994. She performs comprehensive inpatient and outpatient coding reviews to assure correct, compliant coding practices in a multitude of healthcare settings. She is proficient in ICD-10-CM/PCS, ICD-9-CM and CPT/HCPCS coding and multiple reimbursement methodologies including MS-DRG, APR-DRG, APC, Rehabilitation (IRF-PAI), Partial Hospitalization and CMS’s Risk Adjustment program. 

She also performed documentation gap analysis for ICD-10-CM coding during the Centers for Medicare and Medicaid’s (CMS) implementation phase in 2015 and 2016. Ms. Pickering also assists healthcare facilities by reviewing accounts subject to scrutiny by the Recovery Audit Contractor (RAC), as well as preparing responses and/or appeals to inquiries when warranted

Her experience allows her to offer education webinars for coders of various backgrounds and lead and participate in exit interviews with hospital administration to discuss the findings, observations, and operational risk impacts. Attendees of those meetings often include compliance officers, compliance professionals, finance executives, HIM management and staff. She also possesses skills to conduct data analysis to identify trends with help from Information Technology experts. She also provides live and remote webinars to large audiences at state association conventions.
Ms. Pickering is a member of the American Health Information Management Association, and she completed a Medical Record Technology independent study through the AHIMA in Chicago, IL.
